By enrolling in and paying for the below membership, I hereby agree to participate in the Smithville Parks and Recreation Department Indoor Recreation Center. I certify that, to the best of my knowledge, I am/or testify that the minor mentioned below is physically fit and able to engage in any and all activities held within the Smithville Indoor Recreation Center. I agree to indemnify and hold the City of Smithville and its employees harmless from any liability, loss, cost, or expense (including but not limited to attorney’s fees, medical, and ambulance costs) that I may incur as a result of my participation in the Smithville Indoor Recreation Center. In case of any emergency, I give my permission for emergency medical treatment. This statement is also valid for any minors that I allow to participate. My payment acknowledges that I understand and agree to the above conditions.

General Building Rules and Regulations

1. Smithville Indoor Recreation Center Staff reserves the right to rule on any and all undesirable conduct not covered herein and to act accordingly.
2. No smoking, snuff or tobacco chewing shall be allowed.
3. No roughhousing or profanity shall be allowed.
4. All weapons, firearms or fireworks or any other explosive devices are prohibited from the premises.
5. Appropriate footwear shall be worn at all times.
6. Participants shall wear shirts at all time while using the Recreation Center.
7. Staff instructed programs have first priority in facility usage.
8. All persons under the age of 8 shall be under direct supervision of an individual 16 years of age or older while on the premises.
9. Participants must produce proper identification upon request by a staff member.
10. Misuse, abuse, and/or damage to the facility shall be grounds for suspension.
11. Bicycles must be placed in the rack provided before entering the facility.
12. Skateboards and scooters are not allowed in the facility.
13. Office phones are to be used for business only. Public must use provided phone for outgoing calls.
14. Everyone utilizing the gym, weight room, or racquetball court must check in at the front desk.
15. During school hours, no school age children shall be allowed in the Recreation Center without authorization from the school or parent.
16. Alcoholic beverages are not permitted in or around the Recreation Center, unless authorized by the Director.
17. Animals and pets shall not be permitted inside the facility. An exception is made for animals accompanying the visually impaired and/or for special events.
18. Cleats are prohibited in the Recreation Center.
19. The City of Smithville Parks and Recreation Department is not responsible for lost or stolen items.
20. The Recreation Center Staff shall have the authority to suspend any person(s) for up to one week for the first violation of any of the rules. A second violation will result in a one-month suspension. A third and final violation will result in a one-year suspension of all Recreation Center privileges.
21. Fighting, vandalism of the facility, and theft will not be tolerated and will result in an immediate suspension from the facility and the Police Department will be notified.
22. The Director has the right to refuse service to any person(s).
